Background
We compared dissociative seizure specific cognitive behavior therapy (DS-CBT) plus standardized medical care (SMC) to SMC alone in a randomized controlled trial. DS-CBT resulted in better outcomes on several secondary trial outcome measures at the 12-month follow-up point. The purpose of this paper is to evaluate putative treatment mechanisms.
MethodsWe carried out a secondary mediation analysis of the CODES trial. 368 participants were recruited from the National Health Service in secondary / tertiary care in England, Scotland, and Wales. Sixteen mediation hypotheses corresponding to combinations of important trial outcomes and putative mediators were assessed. Twelve-month trial outcomes considered were final-month seizure frequency, Work and Social Adjustment Scale (WSAS), and the SF-12v2, a quality-of-life measure providing physical (PCS) and mental component summary (MCS) scores. Mediators chosen for analysis at six months (broadly corresponding to completion of DS-CBT) included: (a) beliefs about emotions, (b) a measure of avoidance behavior, (c) anxiety and (d) depression.
ResultsAll putative mediator variables except beliefs about emotions were found to be improved by DS-CBT. We found evidence for DS-CBT effect mediation for the outcome variables dissociative seizures (DS), WSAS and SF-12v2 MCS scores by improvements in target variables avoidance behavior, anxiety, and depression. The only variable to mediate the DS-CBT effect on the SF-12v2 PCS score was avoidance behavior.
ConclusionsOur findings largely confirmed the logic model underlying the development of CBT for patients with DS. Interventions could be additionally developed to specifically address beliefs about emotions to assess whether it improves outcomes.

Background.
Compulsory admission procedures of patients with mental disorders vary between countries in Europe. The Ethics Committee of the European Psychiatric Association (EPA) launched a survey on involuntary admission procedures of patients with mental disorders in 40 countries to gather information from all National Psychiatric Associations that are members of the EPA to develop recommendations for improving involuntary admission processes and promote voluntary care.
Methods. The survey focused on legislation of involuntary admissions and key actors involved in the admission procedure as well as most common reasons for involuntary admissions.
Results. We analyzed the survey categorical data in themes, which highlight that both medical and legal actors are involved in involuntary admission procedures.
Conclusions. We conclude that legal reasons for compulsory admission should be reworded in order to remove stigmatization of the patient, that raising awareness about involuntary admission procedures and patient rights with both patients and family advocacy groups is paramount, that communication about procedures should be widely available in lay-language for the general population, and that training sessions and guidance should be available for legal and medical practitioners. Finally, people working in the field need to be constantly aware about the ethical challenges surrounding compulsory admissions.

Dunes adjacent to the Snow Water Lake playa in Elko County of northeastern Nevada rise up to ~10 m above the playa surface in seven distinct clusters. The dunes are composed of tan silty loam containing calcite, quartz, plagioclase, and dioctahedral clay. Abundances of trace elements, along with relative proportions of quartz and calcite, are distinct between dunes along the north and south sides of the playa, reflecting proximity to streams draining different lithologies in the neighboring mountains. Luminescence (optically stimulated luminescence and infrared-stimulated luminescence) dating of dune crest samples demonstrates that the last episode of dune accumulation occurred in the mid-eighteenth century. Moisture-sensitive tree ring records from a nearby site indicate that dune accumulation coincided with an interval of below-average precipitation immediately following a very wet decade. This sequence is consistent with models requiring wetter climatic conditions to move coarse sediment onto a playa surface, followed by dune building under drier conditions. Younger luminescence ages from a sand-dominated unit exposed in an arroyo cut through the dunes may reflect a wetter, more erosive climatic regime ca. AD 1800. The Snow Water Lake dunes are currently eroding, signaling a reduction in the amount of sediment reaching the playa.

Background
Cannabis use has been reported to be associated with an earlier onset of symptoms in patients with first-episode psychosis, and a worse outcome in those who continue to take cannabis. In general, studies have concentrated on symptoms of psychosis rather than mania. In this study, using a longitudinal design in a large naturalistic cohort of patients with first-episode psychosis, we investigated the relationship between cannabis use, age of presentation to services, daily functioning, and positive, negative and manic symptoms.
MethodClinical data on 502 patients with first-episode psychosis were collected using the MiData audit database from seven London-based Early Intervention in psychosis teams. Individuals were assessed at two time points – at entry to the service and after 1 year. On each occasion, the Positive and Negative Syndrome Scale, Young Mania Rating Scale and Global Assessment of Functioning Scale disability subscale were rated. At both time points, the use of cannabis and other drugs of abuse in the 6 months preceding each assessment was recorded.
ResultsLevel of cannabis use was associated with a younger age at presentation, and manic symptoms and conceptual disorganization, but not with delusions, hallucinations, negative symptoms or daily functioning. Cannabis users who reduced or stopped their use following contact with services had the greatest improvement in symptoms at 1 year compared with continued users and non-users. Continued users remained more symptomatic than non-users at follow-up.
ConclusionsEffective interventions for reducing cannabis use may yield significant health benefits for patients with first-episode psychosis.

The results from a third structure determination by powder diffractometry (SDPD) round robin are discussed. From the 175 potential participants having downloaded the powder data, nine sent a total of 12 solutions (8 and 4 for samples 1 and 2, respectively, a tetrahydrated calcium tartrate and a lanthanum tungstate). Participants used seven different computer programs for structure solution (ESPOIR, EXPO, FOX, PSSP, SHELXS, SUPERFLIP, and TOPAS), applying Patterson, direct methods, direct space methods, and charge flipping approach. It is concluded that solving a structure from powder data remains a challenge, at least one order of magnitude more difficult than solving a problem with similar complexity from single-crystal data. Nevertheless, a few more steps in the direction of increasing the SDPD rate of success were accomplished since the two previous round robins: this time, not only the computer program developers were successful but also some users. No result was obtained from crystal structure prediction experts.

The plasma expansion into the wake of a large rectangular plate immersed in a single-ion, collisionless, streaming plasma has been investigated in the laboratory. Several characteristics of the process involved in ‘plasma expansion into vacuum’ that have been predicted theoretically were observed, including the creation and motion of a rarefaction wave disturbance; the creation and motion of an expansion front; and the acceleration of ions into the wake at speeds above the ion-acoustic speed. The expansion was limited to early times; i.e. a few ion plasma periods, by the combination of plasma drift speed and vacuum chamber size. This prevented detailed comparison with self-similar theory, but results are in good agreement with numerical simulations and other laboratory experiments for the early time expansion. The conclusion is that the plasma expansion process is the dominant wake filling mechanism in the near wake of a body, whose potential is approximately the plasma space potential.

Recent laboratory measurements of plasma expansion in a plasma wake experiment are compared with analytical expressions which approximate the plasma expansion model of Crow, Auer & Allen. Good quantitative agreement was found between the data and theory for the velocity and position of the ion expansion front. These results provide an important insight into the behaviour of the expansion early in its development.

The motion of two drops in a uniform electric field is considered using the leaky dielectric model. The drops are assumed to have no native charge and a dielectrophoretic effect favours translation of the drops toward one another. However, circulatory flows that stem from electrohydrodynamic stresses may either act with or against this dielectrophoretic effect. Consequently, both prolate and oblate drop deformations may be generated and significant deformation occurs near drop contact owing to enhancement of the local electric field. For sufficiently widely spaced drops, electrohydrodynamic flows dominate direct electrical interactions so drops may be pushed apart, though closely spaced drops almost always move together as a result of the electrical interaction or deformation.

The ALFA mission is designed to map the entire sky at frequencies between approximately 0.3 and 30 MHz with angular resolution limited by interstellar and interplanetary scattering. Most of this region of the spectrum is inaccessible from the ground because of absorption and refraction by the Earth’s ionosphere. A wide range of astrophysical questions concerning solar system, galactic, and extragalactic objects could be answered with high resolution images at low frequencies, where absorption effects and coherent emission processes become important and the synchrotron lifetimes of electrons are comparable to the age of the universe.
